Remote Patient Monitoring Specialist

We are looking for a highly skilled Remote Nurse or Monitoring Specialist who will monitor patients’ vitals and determine whether they are in or out of range.

· Formal education as a Registered Nurse or Health care aide or equivalent experience (RN, LVN, LPN).

· Strong verbal and communication skills in English.

· In-depth understanding of disease processes related to vital sign monitoring (hypertension, heart failure, CKD, asthma/COPD and obesity).

· Time management and prioritization skills.

· Assessment and critical thinking skills.

· Clinical experience in inpatient settings is preferred.

Tasks:

· Focuses in conducting remote physiologic monitoring treatment management services, clinical staff/physician/other qualified health care professional time in a calendar month requiring interactive communication with the patient/caregiver during the month (99457)

· Call patients for verbal assessment and health teaching.

· Manages computer/on-line guidelines for the purpose of responding to customer queries.

· Coordinating patient care and collaborating with other members of the healthcare team through electronic medical records

· Monitor vitals and determine whether they are in or out of range.

· Assess vitals and prioritize based on patient needs.

· Maintaining patient confidentiality and respect for patient rights in an online environment

· Continuously monitoring and evaluating patient progress and responding to alert cases through remote monitoring tools.
